Every spring, the Department of Neurology under the leadership of the chief freelance neurologist of the Health Department of the Turkestan region, candidate of medical sciences, professor, head of the department of neurology, psychiatry, rehabilitation and neurosurgery of the South Kazakhstan Medical Academy N.A. Zharkinbekova. invites famous scientists from the CIS countries and Kazakhstan to discuss current issues in neurology.
The theme of the current international conference - “Autoimmune and inflammatory diseases of the central nervous system in the Southern region of Kazakhstan” - was not chosen by chance. Since inflammatory diseases of the central nervous system (CNS) occupy an important place in modern neurology. World practice shows that autoimmune processes, such as multiple sclerosis, CHF and others, can be triggered by bacteria, fungi, protozoa and viruses. Which subsequently become the causes of inflammation of the nervous system.
Thanks to focused work, in 2019, a Multiple Sclerosis Cabinet was created at the Regional Clinical Hospital, where patients with autoimmune and demyelinating diseases of the nervous system are provided. Enormous support in those years was provided by the health department of the Turkestan region (whose head was the famous scientist, doctor of medical sciences, professor M.K. Egizbaev). The Multiple Sclerosis Cabinet was created with the goal of improving therapeutic and advisory care for patients with autoimmune and demyelinating diseases of the central nervous system.
An Expert Council has been created in the office, at whose meetings the diagnoses of such demyelinating diseases as MS, CHF, MG and CIDP are verified based on laboratory and instrumental diagnostic methods. It should also be emphasized that the tactics of patient management are completely different depending on the diagnosis. For a long time, the diseases were studied in a single large group, since their clinical picture was quite similar and differed only in pathogenesis.
Statistical data show that 186 patients are registered with the neurological service of the Turkestan region and the city of Shymkent, of which 138 are with multiple sclerosis, 48 patients are with neuromyelitis optica spectrum disease, who receive very complex therapy with DMT (drugs that change the course of multiple sclerosis – first and second lines). 78 patients with myasthenia gravis are being treated. Often these patients suffer from a severe course of the disease, and are treated in intensive care units at the Regional Clinical Hospital.
What does a doctor do in a special Multiple Sclerosis Room? What kind of work does he do? The doctor, as part of the consultation, registers patients, controls treatment, conducts multiple sclerosis schools for neurologists and relatives of patients together with employees of the department of neurology, psychiatry, rehabilitation and neurosurgery of the South Kazakhstan Medical Academy. But the prescription of DMTs (drugs that change the course of multiple sclerosis - first and second lines) is carried out only at a joint consultation of doctors. In the hospital, plasmapheresis, rehabilitation treatment, physiotherapy, physical therapy, intravenous administration of immunomodulators, and administration of DMTs through infusion pumps are carried out for South Kazakhstanis.
And the topics of the reports are devoted to the analysis and prospects for the development of care for patients with autoimmune and inflammatory diseases of the nervous system in the southern region of Kazakhstan, the impact of early initiation of therapy with highly effective DMTs (drugs that change the course of multiple sclerosis - first and second lines) on long-term outcomes in multiple sclerosis. And also - issues of pregnancy in multiple sclerosis, selection of criteria for optimal treatment of multiple sclerosis are considered. It also draws attention to a modern view of myasthenia gravis, case analysis of neuromyelitis optica spectrum diseases, theory and practice of differential diagnosis of demyelinating diseases, laboratory diagnosis of demyelinating diseases of the nervous system, myasthenic crises and clinical observations. The format of the conference was an interactive discussion after each report.
